September 4th Update

From Mary Farris, Assisted Living Manager

Hello Oasis Residents, Families and Friends,

We know that you have missed seeing your loved ones during this pandemic. We are happy to announce that we will be able to allow a modified visitation with your loved one. It is very important that we are consistent with the Arizona Department of Health Services guidance issued on August 28, 2020.  We want to provide the opportunity for you to interact with your loved one in a responsible and safe manner.

Effective September 10, 2020 Oasis Assisted Living and Memory Care will allow visitation with residents when the following conditions are met:

  • Maricopa County community spread level of COVID-19 is at moderate or minimal as determined by the Arizona Department of Health Services AND there is no outbreak (defined as two or more laboratory-confirmed COVID-19 cases among residents or staff) in previous 14-day period.
  • Visits are by appointment only by calling Mary Farris at 480-840-4560.
  • All visits are limited to one (1) visitor at a time for forty-five (45) minutes maximum.
  • Appointments are subject to cancellation depending on the status of community and facility benchmarks.
  • Visits will be limited to a designated visitation area and will not be allowed in the resident’s room.
  • Visitors will be required to provide confirmation of a negative COVID test less than 48 hours old and sign an attestation that they have isolated in the time between when the test taken and the visit.
  • If negative Covid test is not possible, an outdoor visit may be arranged based on outdoor temperatures being safe according to the resident’s treatment plan and individual medical condition and staff availability.

We will continue our current visitation room visits that will NOT require the negative Covid-19 test. You may schedule a visit time by calling Mary Farris at 480-840-4560.

Visitors in all areas must:

  • Be symptom free; see symptom list below,
    • Wear a mask, one will be provided if needed
    • Social distance at least 6 feet during visit.
    • Have a temperature check before entry and must be below 100.4 degrees.
    • Use hand sanitizer upon entry
    • Sign the visitation schedule for contract tracing purposes

Staff will continue to have brief health screening including temperature check, required to wear face mask and do enhanced cleaning and sanitation of the facility where the visit occurs.

Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms may have COVID-19:

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

Your patience and cooperation is appreciated as we work through the process of safely providing visitation with your loved one.

August 27 Update

From Mary Farris, Assisted Living Manager

Hello Oasis Residents, Families and Friends,

Thank you for all of your continued support and understanding during this time. Please note that visitation restrictions have NOT been lifted for Assisted Living Facilities.  We must remain in compliance with Arizona Department of Health Services regulations as they are implemented/adjusted for the protection of your loved ones. At this time we have no signs, symptoms or positive results here at The Oasis Assisted Living and Reflections Memory Care.

Visitation to the facility is limited to ESSENTIAL SERVICES AND END OF LIFE VISITORS ONLY.

In light of this I wanted to touch base on some items for the visiting room here at the Oasis:

  • Call the lead caregiver (480-318-5576), Mary (480-290-7027), or the Front Desk (480-290-7903) ahead of time when possible to schedule your visit. (This can also be done ahead of time for later dates in the week)
  • Be respectful of other visitors and their time with loved ones.
  • If you show up with no notice you may have to wait for an opening. Those who schedule ahead of time will be given the time slot.
  • Visits are 15-minutes and start from the time both parties are present at the visiting window.

Precautions we are taking:

- Any staff, essential services and end of life visitors will continue to have a small health screening including a temperature check before entering the facility.

-      Masks or cloth face coverings are mandatory for all staff. Handwashing protocols are being reviewed frequently.

-      Packages, care items, food and deliveries can all still arrive at the facility.

-      Residents are able to travel to doctor’s appointments as necessary.  We do ask them to wear a mask when going.

-      If you are personally dropping off the items please leave them with the screener at the table; if one is not available that time of your drop off please call the lead phone at 480-318-5576.

-      All residents are monitored daily for any signs or symptoms of COVID-19. At this time we have no confirmed positive residents or staff members in the Oasis Assisted Living and Reflections Memory Care.

In light of this ruling we have a couple alternatives to offer for communication:

  • A beautiful, temporary visiting window has been placed in the dining room to provide visitor and resident comfort and safety from the Arizona heat. Visiting hours of 9am – 5pm will apply to this area of the building.
  • Video chats and phone calls can be set up by staff members.  Please contact me to schedule this item ahead of time.

Separation can be very hard and emotional for both parties. Thank you so much for all of your patience and understanding during this time.  In an effort to keep residents fully engaged we have:

-      Activities performing one-on-one visits with residents in their apartments.

-      Very limited, socially distanced group activities are available multiple times a day.

-      SeniorFIT helps residents keep moving and active with one-on-one visits in their apartments.

-      Meals are delivered by dining staff 3 times a day to resident’s apartment.

If you need assistance with contacting your loved one via video chat or phone call please don’t hesitate to contact me and we can set up a time that works best.

May 6 Update:

Following is the latest communication from Assisted Living Manager, Mary Farris:

Hello Oasis Families and Friends!

We know these are challenging and unprecedented times. We appreciate your cooperation through this process. Thank you for all of your patience while we maneuver through the frequent changes.

As Arizona begins to open up various public businesses, please note that our community continues to adhere to the Coronavirus guidelines from the CDC, AzDHS and Maricopa County Department of Public Health. At this time the “opening” does not apply to our Assisted Living and Memory Care – we are still only allowing essential visitors and end of life visitors at this time.

Any packages, food, flowers etc. can be dropped off at the door of the facility with one of the staff members. If you do not see someone stationed at the door please call the lead caregiver at 480-318-5576.

If you need assistance with a video or phone call to a loved one, please email me at mary.farris@christiancare.org.

April 30 Update:

Following is a communication from Assisted Living Manager, Mary Farris ...

Hello Families and Friends!

Let me start by thanking you all for your patience during this unsettling time. We have received COVID-19 test results for all of the Oasis Assisted Living & Reflections Memory Care and I am happy to report that zero (0) positive results were found. Please keep in mind that the test performed is good for that moment in time but, our community continues to adhere to the Coronavirus guidelines from the CDC, AzDHS and Maricopa County Department of Public Health.

For your additional peace of mind, here are some additional precautions we are taking:

  • Residents are being monitored daily with a temperature, pulse and oxygen saturation % readings and they will be monitored for any signs of respiratory illness.
  • All staff has a brief health screening when they report to work and they are required to wear a cloth face covering and picture ID.  All staff has been trained on proper infection control procedures.
  • Visitors are still limited to essential service providers or end of life visitors only, they must pass a small health screening before entry to the facility is granted.
  • Deliveries can be dropped off at the door of our Assisted Living building.  The screener is actively at the door from 5am – 5:30pm.  For times outside of that window please contact the lead cellphone at 480-318-5576.

At this time we are still on lockdown until the Coronavirus guidelines from the CDC, AzDHS and Maricopa County Department of Public Health are updated.  Even though you cannot visit your loved one in person at this time we are able to keep communication open. If you need assistance with calling or video conferencing a resident please let me know and I can set up a time with you to make that happen.

April 7 Update:

The good news is that at this time, we have ZERO confirmed cases of Covid-19 in our Independent & Assisted Living communities as well as Memory Care. That being said, with the spread of the virus predicted to be near it's peak, we want you to know that we have specific protocol in place should we become aware of illness of a resident or staff member. These procedures will help ensure safety of staff and residents while continuing to provide good care.

March 31 Update:

Following is a letter from Mary Farris, Assisted Living Manager to the family and friends of our residents:

Greetings friends and families,

To ensure that everyone is on the same page with all of the sudden changes I wanted to send out an overview of our current precautions in place.

At this time our entire campus (Independent Living, Assisted Living and Memory Care) is closed to everyone except:

Essential services or end of life visitors only until further notice.

Below is information that outlines what this means too our residents and their families:

Oasis Assisted Living Residents:

  • Residents returning to our community (this should be only for urgent/essential medical appointments) will be stopped and a health screen will be performed in addition to the screening that is performed each morning for every resident. If weather is not ideal please have them stay in the vehicle for the screening.  You can let the attendant know - If they are not at their table please call the lead cellphone at 480-318-5576.
  • Packages & items can be dropped off at the front door with the attendant – If they are not at their table please call the lead cellphone at 480-318-5576.

All essential services visitors (i.e. hospice personnel, home health, physical therapy, medical equipment companies, emergency services)

  • Will be cleared by a health screening before they are able to enter the facility (This does NOT apply to emergency services as they could be responding to a life-threatening event and have their own screening in place at their assigned stations).
  • Essential services visitors must clear the CDC guidelines including a temperature of 100.4 or less degrees and no symptoms of respiratory infections.

Personal Deliveries

  • Delivery will be accepted from drivers or family/friends at the front door by a staff member.
  • Our team will get the delivery items to the resident’s apartment.

Please note that if you are unable to comply with the above restrictions the option to take your loved one home with you is available but, they must remain in your care for the duration of the mandated lockdown.

Activity Update: A couple items have been implemented to battle the loneliness that some may be feeling at this time.

  • Hallway concerts that residents can enjoy from their doorways.
  • SeniorFit one-on-one visits with willing participates. Contact Information: Matthew Clark - Matthew.Clark@christiancare.org

March 20 Update:

The Centers for Disease Control and Prevention (CDC) have identified older adults as high risk for complications from the Coronavirus (COVID-19).  The federal government is urging older people to “stay home and away from other people” for 15 days to stop the spread.   At Fellowship Square Historic Mesa it has been through much prayer and concern for our residents' health that we have made some difficult decisions that we know impact their daily lives.  With that in mind the following changes are being made:

Our Oasis Assisted Living and Reflections Memory Care communities remain on lock down.

We will be setting up a temporary smoking section in one area of the center courtyard.

Transportation: Only individual transportation for medically necessary appointments will be provided.

Activities: Group activities are temporarily halted so we are limited to 1 on 1 activities.  Matt with SeniorFit will be going door to door to see who he can help enjoy some exercise.

Please remember that no one comes in or goes out of the building unless it is medically essential.

March 19 Update:

Dining:  As directed by the Arizona Department of Health Services, Assisted Living will be going to meal delivery only as of today.  There will be one standard Breakfast and when delivered, the staff will take their lunch & dinner orders.  The menu will remain the same for now.

This information is in the process of being communicated to residents.

Meal Delivery Schedule:

Breakfast:

First Floor: 7:30am to 8:15am

Second Floor: 8:15am to 9am

Lunch:

First floor: 11:30am to 12:15pm

Second Floor: 12:15pm to 1pm

Dinner:

First Floor: 4pm – 5:15pm

Admittance:  The Oasis remains in lock down – no visitors.  Only required medical personnel.  ANYONE, in addition to staff already being done,  who wants to go beyond the lobby entrance will have to have their temperature taken and complete the health questionnaire.

Activities – continuing with groups of 10 or less only.

Our staff are going above and beyond to help residents become as comfortable as possible with these sudden changes.  We appreciate them so much!

March 17 Update:

To be clear, we do NOT have any residents or staff, in our assisted living, or on our campus, that have the signs and symptoms of COVID-19, or Coronavirus. However, we want to take every precaution, in advance of any potential local escalation of the virus, to limit the potential exposure and illness that could potentially spread to our campus and affect our residents.

As precautions, we have done the following:

  • We are not allowing any outside visitation, until further notice unless approved by Assisted Living Manager for medical reasons.

Why? The safety of our residents is our top priority. We want to limit as much potential exposure as possible. We will make every effort to keep you updated on your loved one’s condition and daily activities.  To drop off packages or pick up laundry, push the white button on the left of the front entrance to call the Lead Caregiver or call 480-318-5576 to exchange outside the front door.

  • We are screening all staff daily.

Why? As a healthcare facility, we are responsible for the health and welfare of all residents and staff. We are taking temperatures and asking information from all staff about their potential exposure.

  • Our Assisted Living Manager will utilize email and emergency text notification system for updates.  To receive text alerts, please text hmoasis to 602-603-4431. No cost to enroll & normal text messaging rates apply. Mary can be reached at Mary.Farris@ChristianCare.org or by calling 480-290-7027.

Why? As we communicate further about our status and update you on any changes, this will ensure timely notification.

March 12 Update:

In response to Governor Ducey’s declaration of a public health emergency and executive order of March 11th pertaining to COVID-19 (aka novel corona virus), Fellowship Square Historic Mesa has taken numerous steps to proactively help protect our senior living residents from the virus.

Visitors: Visitor arrival times are limited from 9 am to 5:30 pm, 7 days a week.

Individuals should not attempt to visit if they have respiratory infection symptoms including: Fever, Coughing, Shortness of Breath, Difficulty Breathing, Chills, Body Aches, Sore Throat, Headache, Diarrhea, Nausea/Vomiting and Runny Nose.

Visitors will be asked and required to respond to health questions upon arrival and will be denied access if they exhibit any symptoms or respond positively to any of the questions.

If family are not able to visit, we will do our best to accommodate visits via telephone and electronically.

Activities & Events: All activities involving outside vendors, volunteers or visitors are cancelled until further notice. We will do our best to step up staff coordinated activities during this time.

Staff: All staff will be subject to the health checks before and during their shift. If they have symptoms of respiratory infection, they will be sent home and will be required to show a negative test result before they are allowed to return to work.

Our Emergency Coordinator is our Assisted Living Manager, Mary Farris.

Mary will provide updates via our N.O.R.B.E.R.T. text alert system. You can access the Oasis text alert system by texting “hmoasis” to 602-603-4431. There is no cost to enroll. Normal text messaging rates apply. Text “stop” to unsubscribe.
