For our residents’ comfort and convenience, Christian Care is pleased to provide Home Health services at Fellowship Square Historic Mesa.

Home Health services are usually started after a critical medical/health event, but can also be a great way to deter declining health by providing tools needed to improve health status.

We offer the following services:

  • Skilled Nursing
  • Wound Care
  • Ostomy & Catheter Care
  • IV Infusion Assistance
  • Pain Management
  • Medication Management
  • Certified Nursing Assistance (showers, light housekeeping, etc).

Bringing these services to the residents in their homes at our senior assisted living campus helps ensure that they will get the care they need, when they need it. It removes a common barrier to receiving care by reducing the stress and struggle of getting to certain medical appointments. It also helps relieve family members from having to take so much time off of work to get their loved one to these appointments.

Most individuals can benefit from Home Health services, whether it is therapy to help improve balance, gait or strength or nursing to help teach about a disease process. We can even advise on diet concerns or how medications interact and how to safely take them.

If we note that a resident in our assisted or independent living community is experiencing falls (from our Security incident reports), we can contact their primary care physician and request an order so that they can begin physical or occupational therapy. By being proactive in this way, we can help prevent additional falls.

We are Medicare certified and accept a variety of insurance plans such as traditional Medicare, United Health Care and Blue Cross/Blue Shield of Arizona. Residents must have a medical necessity and a physician’s order to receive the skilled services of a nurse or therapist.

In addition to being Medicare certified, we are also licensed by the State of Arizona and accredited by the Joint Commission. These health care and insurance options allow for our residents to plan for the cost of long term care when it comes to determining options for senior living in Mesa, AZ.
