Wellness Connection Helps Residents Maintain Independence

It’s practically unavoidable.  At some point in time, all of us or someone we care about, will be challenged with activities of daily living.  This happens as a consequence of aging or disease and the associated decline of vision or hearing, mobility, balance or energy.  When people cannot keep up with activities of daily living, not only can physical health suffer, but they can become isolated and depressed.

Some of the first activities of daily living that people find challenging include keeping up with housework and yardwork, driving and getting to appointments or even cooking nutritious meals.  Struggling with these types of things can add a great deal of stress to an individual’s life and for those who care about them. Because of this, it is really important that families think ahead and become educated on senior living and care options so they can transition to living situations that meet their needs as they age.

To coincide with our Mission, Values & Vision and to support our resident’s changing needs, Fellowship Square Historic Mesa recently organized a variety of services under one umbrella and has dubbed it the “WELLNESS CONNECTION”.

•  Supportive Community Living Options To help our residents “Age In Place” in a community with staff that families know and trust.


o Independent Living is for seniors who are still quite active but need just a little help and want more time to enjoy the things they want to do – not the things they have to do.  Services include dining, housekeeping, security, emergency alert and daily assurance check system, transportation and activities.


o Assisted Living - A higher level of care for individuals who need more regular assistance with activities of daily living.  Residents are still able to live in their own private apartment and have 24 hour access to caregiver support.


o Memory Care - For individuals who are struggling with dementia; the deterioration in memory, thinking, behavior and the ability to perform everyday activities.  

Our supportive living communities help our residents enjoy life and live with purpose to the fullest extent possible.  Unfortunately however, too many of our residents can still struggle and need more help than we had been able to offer them in the past.   Our Wellness Connection services help fill in where additional support is needed; better aiding them in their quest to maintain their independence.

o Home Health: Medicare certified skilled medical home health services in the convenience of their home.  Services include medication management, skilled nursing consultations and more.


o Supportive Services: For help with non-medical in home care needs such as shower/dressing assistance, campus orientation/escorts, assurance checks and more.


o Outpatient Rehab:   Offering on-site physical, occupational & speech therapies including muscle strengthening, walking and balance training, fall prevention, pain management and more.


o Dr. House Visits:  Physician Services with on-site clinic hours or visits in your own home.


o Aviant Palliative Care – specialized in home medical care focused on providing relief from systems and stress of serious illness.


o Senior Fitness: Health and fitness programs designed especially for seniors under the supervision of a trained fitness instructor.


o Abundant Life: On-site and in home visits providing individualized holistic nutrition coaching.


o Education and Support:  A variety of support groups meet regularly to discuss diabetes, nutrition, Parkinson’s, cancer, caregiving and more.

At the center of all of these services is our Campus Care Coordinator, Mitch Saavedra.  Mitch’s job is to consult with residents and family members and help them get set up with the care services that will be most beneficial to the resident.  Mitch can be reached at 480-290-7019.
